Hospice is a program of care that provides physical, emotional, spiritual, and financial support to terminally ill patients and their families.

Hospice is a philosophy of care, a supportive alternative for those whose life expectancy is limited by illness. With the help of Hospice, patients can remain in their own homes surrounded by family and friends. The goal of Conemaugh Regional Hospice is to maintain a high quality of life, and to provide comprehensive physical, emotional, spiritual and financial support to patients and their families. We are dedicated to ensuring that patients are treated with dignity and compassion.

THE INTERDISCIPLINARY TEAM

OUR TEAM

Hospice care is provided by a team of professionals including a medical director, the patient’s attending physician, nurses, social workers, counselors, home health aides, trained volunteers and clergy. Nutritionists and speech, occupational, and physical therapists are also called upon as needed. The Team meets regularly to evaluate and coordinate each patient’s care in the home.

HOW ARE REFERRALS MADE?

REFERRALS

A patient, family member, physician, public health nurse, social service worker, clergy, friend or neighbor may call the Conemaugh Regional Hospice office. Hospice care is provided without regard to race, color, age, sex, religion, national origin, handicapping condition, or ability to pay.

PATIENT CARE

PATIENT CARE

Care at home enables patients to live their lives with independence and dignity. A family member or other caregiver tends to the daily needs of the patient with the support and guidance of the Hospice Team. Although Hospice care is centered in the home, it is sometimes necessary for the patient to receive care for a short period of time in a hospital setting. The Hospice Team will help arrange for this care and will stay involved with the patient and family, resuming care when appropriate.

SYMPTOM CONTROL

WE TREAT THE SYMPTOMS

Hospice Care treats the symptoms of the disease, such as pain, constipation, nausea, vommiting and shortness of breath. Controlling these symptoms allows the patient to live comfortably, spending precious time with family and friends.

AVAILABLILITY OF SERVICES

OTHER SERVICES

Besides symptom control, Hospice also provides:

Emotional Support, Counseling, Access to Community Resources, Spiritual Support, Caregiver Teaching, Bereavement Follow-up

The Conemaugh Regional Hospice program provides coordination of care for patients and their families 24 hours a day, seven days a week.

WHO PAYS FOR HOSPICE SERVICES?

COST OF SERVICES

Hospice care is usually covered by a patient's medical insurance.  However; if the insurance does not have a hospice benefit or if the cost of his/her care exceeds what the insurance will pay, Conemaugh Regional Hospice does not charge the patient for any costs. No one is denied services due to inability to pay.  Hospice will pay for any medications that are directly related to the patient's terminal illness or any medications that are managing symptoms that stem from the diagnosis.  Hospice will also pay for any basic medical equipment that a patient may need and respite care services for caregivers.
